What can the factory do to reduce the high defect rate ?

1. Analyze the root causes
Fishbone method is normally used to analyze the root causes, the factors could be 5M1E (Man, Machine, Material, Method, Measurement, Environment). Attached illustration is FYR.

2. Determine the priority
FEMA (Failure Mode and Effects Analysis) is usually used to determine Risk Priority Number (RPN) for Potential problems, however, to my point of view, it is also can be applied for an existing problem.
RPN = S (Severity) * O (Occurrence) * D (Detection)

3. Implement the measures
Pick the factors with higher RPN taking into the account of the costs, and implement the actions accordingly referring to 5W2H (What, Why, Who, When, Where, How, How much).
